The Disaster Information Form, delivered during the Wild-land Fire Exercise along with several pieces of other information, requires action on your part. The information requested on the form will allow us to aid you in an emergency. This is an updated version of a previous form you may have filled out in the past. If you filled out the form before, and there have been no changes, and you confirm with your Neighborhood Leader that he or she has the form take no action. If you have changes and have the original form make them on that copy. If you have not filled out the form before or you want to start from scratch use the copy delivered to you. Under vehicles-Cargo we want to know who has room in their vehicle to transport a 72" long backboard with patient.
Deliver the form to your Neighborhood Leader so that they will have the information in an emergency. If you don't know who your Neighborhood Leader is look towards the back of your directory on the map, click here, or contact Bill Tagg.
Don't forget to include a drawing of your site including your gas, water, hose bibs, and fire sprinkler (if any) shut off valve locations.
Show main electrical disconnect. Indicate street, driveway, gates etc.
